Important Information about this website
This site is being
continually developed and updated. All of the work and cost of producing and
maintaining this web site has been provided personally by the Doctors as a
service to our patients. We have not received any NHS funding or assistance and
the work is done in our own free time. We hope that you find this facility
useful and your comments are very important to us. We would welcome any
suggestions on improving our web site. However, due to time constraints, it is
not anticipated that we will be checking and responding to any e-mail on a
regular basis. If you contact us via this web site, there may be a long delay
before you receive a response.
You
are advised to discuss any of the information given at this site with your
own Doctor beforehand. We cannot accept responsibility for any errors. It
is your responsibility to decide whether to act upon the information given
at this web site. The information given is intended ONLY for patients
living within our practice area and registered with us. If you do not live
within our practice area and are not registered with us, please do not
proceed further. The authors of this website will not accept
responsibility for any consequences arising from the advice given within
this website. Although we have taken great care in preparing this web
site, we would like to advise you that any material obtained from the
Internet may not have been verified and could even be harmful. Please
consult your Doctor if you have any medical problems or concerns.
The
information contained on this site is general information which is not in
any way intended to replace the specific advice that patients may obtain
from their own doctor about individual care.
This
site does not contain any confidential material, patient details or
clinical records. It is not possible to access confidential information,
patient details or clinical records via this web site. Please do not send
any confidential or private information to us via this web site or via the
Internet. The hardware used to maintain this site is totally separate and
not connected in any way to our clinical systems.
Unfortunately, we are unable to respond to e-mail of a personal nature.
Your e-mail may be seen by non-medical staff and we cannot guarantee your
confidentiality. With the exception of online repeat prescription
requests, e-mail sent via this website is received at a distant
location and is not checked regularly nor forwarded to individual Doctors.
For this reason, we ask that you do not send any personal
questions for Doctors.
Online
prescription requests:
Only items listed on your repeat prescription side slip can be ordered.
Items can only be ordered when they are due.
If incorrect patient details or drug names are
submitted, your order will not be processed by our staff.
Any requests for items which are
not listed on the repeat prescription will not be processed. Please type in the medication
names with the exact spelling and doses as shown on your repeat prescription
slip. Please note that the name on the tablet box may be a "brand
name" which is not necessarily the same name as the one on your repeat
prescription slip - it's the name printed on the repeat prescription
slip that we want! You must select the correct surgery
otherwise your request will not be processed quickly and there may be a delay in
getting your medication. Confirmation - Each request is confirmed in the form of a confirmation page
that opens automatically in your browser after hitting the
"submit" button. This page confirms your details and your order.
This confirmation page verifies that the details you entered on your
request form have been e-mailed from the web server to the surgery.
It DOES NOT verify that we have received it. If there are any
problems with the NHS email service, it is possible in exceptional
circumstances that your details may not reach us. After
pressing the "submit" button, you should wait for this
confirmation page to appear
before moving on (depending on the speed of your Internet connection,
this confirmation page should only take a second or two to load). Please
note that your request is not confirmed by e-mail. We recommend
that you print and keep a copy of each confirmation page every time you
order.
Confidentiality of Patients Records - A Guide to Practice Accreditation
Visits - Joint Committee on Postgraduate Training for General Practice
Your records may be disclosed to persons outside the primary care team
which provides their care. The purpose of such disclosure is strictly limited to
a verification process which is required to ensure practice compliance with
educational standards for trainee doctors. The visit team are all subject to a
strict duty of confidentiality. Any patient may object to the whole, or any
part, of the inspection of their own medical records and that their objection
will be respected.
